*Note: In order to purchase a menu, you must first log into your account. If you do not have an account, you must register an online account.

Step 1: Create and Activate Account - After you successfully registered an online account, you will receive both a Welcome email confirming your registration and another email containing an Activation Link. Once you have activated your account, you will be able to continue the ordering process. Note: You will need to have access your email account.

Step 2: Purchase an Online Menu(s) securely online using:

Step 3: Fill Out Online Evaluation - You will be asked to fill out information regarding your new menu request (i.e History, symptoms, time commitment).

Step 4: Send your digital photos via email - Simply send us your digital photos.

Requirements:

Computer with an internet connection
access to local email account (i.e. Outlook, Eudora, Mac Mail, etc.) or web-based email account (Hotmail, Yahoo, etc.)
Digital camera
4 digital pictures (Front, Back, Left, Right).
Photos must be emailed in .jpg or jpeg format and should be no larger than 200KB each

Recommendations:

We strongly recommend that Online Therapy participants have basic computer knowledge, or a friend or family member with basic computer knowledge to help them.

What to wear:

T-shirt, tank top, or sports bra
Shorts (we must be able to clearly see your knees in the photo)
Light clothing preferred when taking photos
Bare foot, no shoes if possible
